Survey Guide:

1. The recommended browser for the survey form is Google Chrome.

2. You do not need to provide your personal details while completing this survey form; therefore, please rest assured that your ANONYMITY will be preserved.

3. There are 15 departments to be assessed, as well as an additional survey on 'Whistleblowing Awareness'.

4. You may SELECT the departments that provide services relevant to you by clicking the ‘SELECT’ button. ALL QUESTIONS must be answered before proceeding to another department by clicking the ‘NEXT’ button.

5. If you are uncertain or unable to rate a particular statement or question, please select 'NO OPINION' as your answer.

6. Please DO NOT SELECT your own department to ensure impartiality of the survey exercise.

7. The ‘Whistleblowing Awareness’ section is MANDATORY; hence you need to answer all questions under this section.

8. If you have mistakenly selected a department and wish not to conduct the survey against that department, simply click the ‘BACK TO INTRO’ button at the top of the page.

9. Once you have completed the survey, you may proceed with form submission by clicking the ‘SUBMIT SURVEY’ button in the Whistleblowing section and ‘CONFIRM’ button.

10. After clicking 'CONFIRM', a 'THANK YOU' message will appear, along with a unique CODE. The CODE is necessary if you wish to continue the survey later (in case you need to 'EXIT' without submitting the form) and also to claim the TOKEN which will be awarded to all respondents (while stock last). You will be notified via email later.

11. However, if you do not want to submit your responses, you can click the 'EXIT' button in the Whistleblowing section. You may then return to the survey at a later time to make changes or continue until you are ready to submit the form.

12. You need to screenshot the CODE in order to continue with the same survey.

13. Your responses will not be recorded or submitted until you have completed Step No. 9 and 10 of this Survey Guide.
